Subject: Clicking in one gear

I have a Look 595 with Dura-Ace setup ... and an annoying click that only occurs in the 4th gear regardless of which chainring I'm in. I've tried making an adjustment to the cable on the derailleur but that doesn't seem to do it. I've never had this happen with just one gear like this on my other bikes. Any ideas?

A bent tooth is not uncommon with these very thin cogs, BTW-
