Subject: Rumbling and click

Hoping that the more mechanically-inclined riders could help me with a question. Was riding the other day when my I started getting a rather harsh clicking / chatter when climbing in the lowest two or three gears. I can both feel it in the crankarms and hear it coming from the cassette and I thought that perhaps the chain was jumping and I needed to adjust the barrel adjuster. Tried that and it's not working. I can also feel it when the chain's in the middle of the cassette, but the problem is at it's worst when I'm climbing and standing and really pushing on the pedals. Also, I get a rather intense rumbling / vibration when I'm in the tallest 2-3 gears. It's not a clicking, but definitely some sort of major vibration that I don't experience in any other gears and, basically, I have no idea how to fix it. I would imagine that both issues are related somehow to either the chain, cassette, or derailleur, or some sort of combination of all three. Of course, any insight lended is always appreciated.

And let me also say that when I click through the gears while the bike's on a stand, the chatter / clicking problem is not evident but the high gear rumbling is still there. Just and FYI.
